If you have only received one dose of a two dose vaccine series, you can complete your vaccination attestation form with one dose now and update your form 14 days after you received your second dose.

To assist with planning purposes, we encourage all members of the 杏吧原创 University community to complete the vaccination attestation form and submit their documents as soon as possible even if you are not planning to participate in in-person activities at this time.
