The Thesis Supervisor is responsible for CHOOSING and CONFIRMING the following committee members, including the Chair. The Supervisor submits this form.

Please note the following:

• Instructors are generally not eligible to serve on a defence committee. If an instructor is selected, justification is required and must be emailed to DSAAI@carleton.ca.
• A Chair cannot be from any of the participating DSAAI units. To receive a list of potential Chairs, please email DSAAI@carleton.ca.
• This information must be provided at least six weeks before the proposed defence date.
