Students are not required to share medical information with faculty in order to access accommodations.<\/p>\n\n\n                            <\/header>\n\n                    <\/div>\n\n            <\/div>\n\n    <\/div>\n<\/section>\n\n\n\n<p>To register with PMC you will be required to provide <strong>current medical documentation from an appropriate regulated health care practitioner<\/strong> that contains the following information:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>confirmation of a diagnosed disability;<\/li>\n\n\n\n<li>functional impairments or limitations relevant to the university setting;<\/li>\n\n\n\n<li>duration of disability status (permanent, persistent\/prolong, temporary);<\/li>\n\n\n\n<li>co-existing conditions, if applicable;<\/li>\n\n\n\n<li>recommended course load; and<\/li>\n\n\n\n<li>any other relevant information.<\/li>\n<\/ul>\n\n\n\n<p><strong>Documentation forms available below &#8211; one for each disability category &#8211; have been developed by the PMC to gather relevant information that your PMC coordinator needs to determine appropriate accommodations and support services during your academic studies at ÐÓ°ÉÔ­´´.<\/strong><\/p>\n\n\n\n<p>These forms may be used in-place-of and\/or in conjunction-with existing or past documentation. Please review the documentation requirements for the relevant category, and the respective documentation form(s) below.<\/p>\n\n\n\n<p>If necessary, ask your regulated health care practitioner to complete it prior to your intake appointment.<\/p>\n\n\n\n<h3 id=\"acquired-brian-injury-abi\" class=\"wp-block-heading\">Acquired Brian Injury (ABI)<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A copy of a recent neuro-psychological assessment, if available. <strong>OR&#8230;<\/strong><\/li>\n\n\n\n<li>A PMC <a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/101\/2025\/07\/PMC_Documentation_ABI-1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Documentation of Acquired Brain Injury Form<\/a> completed by a regulated health care professional.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"attention-deficit-hyperactivity-disorder-adhd\" class=\"wp-block-heading\">Attention Deficit\/Hyperactivity Disorder (ADHD)<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A copy of your latest comprehensive psychoeducational assessment, completed by a registered psychologist or psychological associate. An assessment completed within the last 5 years, or at the age of 18 years or older, is preferred. <strong>OR&#8230;<\/strong><\/li>\n\n\n\n<li>A PMC <a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/101\/2025\/06\/PMC_Documentation_ADHD-1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Documentation of ADHD Form<\/a><a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/3\/PMC_Documentation_ADHD-1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"> <\/a>completed by a regulated health care practitioner.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"autism-spectrum-disorders\" class=\"wp-block-heading\">Autism Spectrum Disorders<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A psycho-educational assessment (PEA), completed within the last 5 years or performed using adult-normed tests. <strong>OR&#8230;<\/strong><\/li>\n\n\n\n<li>A <a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/101\/2025\/08\/PMC_Documentation_ASD-1.pdf\">PMC ASD documentation form<\/a> completed by a regulated health care practitioner who can confirm diagnosis and provide information on current functional limitations.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"chronic-medical-conditions\" class=\"wp-block-heading\">Chronic Medical Conditions<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A PMC <a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/101\/2025\/07\/PMC_Documentation_Medical-1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Documentation of Medical Disability Form<\/a> completed by a regulated health care professional is preferred.<\/li>\n\n\n\n<li>If it is not possible to get this form completed in time for your intake appointment, we will accept other medical documents that confirm that nature of your disability and functional impairments relevant to the university setting as interim documentation.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"deaf-and-hard-of-hearing\" class=\"wp-block-heading\">Deaf and Hard of Hearing<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A recent audiogram or audiology assessment report, <strong>AND&nbsp;<\/strong><\/li>\n\n\n\n<li>A PMC <a href=\"https:\/\/carleton.ca\/pmc\/wp-content\/uploads\/sites\/101\/2025\/07\/PMC_Documentation_HE-1.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Documentation of Hearing Impairment Form<\/a> completed by an otolaryngologist, audiologist or treating family physician.<strong>&nbsp;<\/strong><\/li>\n<\/ul>\n\n\n\n<h3 id=\"learning-disabilities-ld\" class=\"wp-block-heading\">Learning Disabilities (LD)<\/h3>\n\n\n\n<p>A recent&nbsp;<strong>Psycho-Educational Assessment (PEA)<\/strong>&nbsp;completed by a registered psychologist or psychological associate within the last 5 years or at the age of 18 years or older is preferred.
