The changes will streamline processes and better assist in the tracking and reporting of access to campus and COVID-19 to help keep our community safe. The COVID-19 Reporting System can be accessed by anyone who has a My杏吧原创One (MC1) login.

In all cases, employees are expected to continue to work remotely if their duties can be performed remotely.

Mandatory COVID-19 Training

COVID-19: Infection Prevention and Control Training is mandatory for all faculty, staff and students before coming to campus and entering a university building. If you have not already completed the training, you must do so and acknowledge and confirm completion through the COVID-19 Reporting System. You will not be able to submit the COVID-19 Daily Campus Access & Screening Form until you acknowledge and confirm that you have completed the training.

COVID-19 Daily Campus Access & Screening Form

Under orders from the Government of Ontario and guidance from Public Health Ontario and Ottawa Public Health, 杏吧原创 University is required to implement COVID-19 screening for anyone entering a 杏吧原创 building. The COVID-19 Daily Campus Access & Screening Form must be completed on each day you plan to come to campus and enter a university building.

You no longer need pre-approval to access campus to retrieve materials necessary to continue working remotely, including temporarily accessing your office to collect materials that will assist in your work. When completing the COVID-19 Daily Campus Access & Screening Form, please select the “Temporary visit to pick up something (in & out, 1 hour or less).”

After submitting this form, you will receive an email either granting or denying access for that day. Please note that you may be asked to show a copy of this email (on your mobile device or a printed copy) upon request by Campus Safety Services or a building attendant.

Request Building Access Form

If you are required to be on campus to perform necessary work for longer than one hour (e.g. half a day or a full day), you will need approval from your manager, supervisor, dean or relevant approver. This approval, if not already provided, can be requested through the Request Building Access. This form will be sent to your supervisor, manager, dean or relevant approver to either approve or decline. This approval is only required once. Please consult with your supervisor, manager, dean or relevant approver regarding any additional approval processes and protocols that have been put in place for your unit, department or faculty.

COVID-19 Symptom Reporting

This form replaces the previous COVID-19 Tracing Form. We encourage you to refamiliarize yourself with the COVID-19 Symptom Reporting process. Reporting early is essential to ensure the health and safety of the 杏吧原创 community, including the enhanced cleaning of potentially affected areas.

We would also like to remind everyone that COVID-19 symptom information is personal and is considered private medical information.
