References from non-academic supervisors may be accepted in certain cases i.e. professional references or for professional programs.<\/p>\n<p>NOTE:\u00a0 References from family members, friends, students, former or current classmates, employees, colleagues and\/or\u00a0Teaching Assistants are not accepted.<\/p>\n<p>Contact your referees before you apply online to confirm their availability.<\/p>\n<p>When filling in your online application, you are asked to include email addresses for your referees.\u00a0<strong>NOTE: Valid institutional or organizational email addresses are required. The statement of intent is to be written in consultation with the proposed faculty supervisor from the Department of Health Sciences. Please see the <a href=\"https:\/\/carleton.ca\/healthsciences\/current-students\/forms-guidelines-and-documents\/\">Forms page<\/a> for the <strong>Thesis Research Proposal (Statement of Intent) and Supervisor Agreement Template for HS MSc and PhD<\/strong>. This 1-page document must include: <strong>1)<\/strong> an abstract of the proposed MSc research thesis project (background\/rationale, research question(s), aims) and <strong>2)<\/strong> supervisor information (name, contact information, and statement from the proposed supervisor acknowledging commitment to supervise and fund the student for the proposed thesis project). Both proposed supervisor and student must sign this form.<\/td>\n<\/tr>\n<tr>\n<td>CV\/Resum\u00e9<\/td>\n<td>One (1) is required. \u00a0This must include an <strong>up-to-date CV<\/strong> of the applicant, with a <strong>Statement of Experience<\/strong> highlighting the student\u2019s specific skills in research design, analysis, and writing; their willingness or ability to work collaboratively or in interdisciplinary teams; and their expertise, previous research, and\/or work experience that positions the student to be successful in the interdisciplinary program.<\/td>\n<\/tr>\n<tr>\n<td>Data Science Specialization Form (MSc only)<\/td>\n<td>One (1) is optional. You will need to complete the <a href=\"https:\/\/carleton.ca\/healthsciences\/wp-content\/uploads\/Data-Science-Specialization-Form-2.pdf\">Data Science Specialization Form<\/a>\u00a0if you are interested in applying to the collaborative master\u2019s program in Data Science.
