I acknowledge that the intention of this event is for women students with multiple intersecting identities.

The Faculty of Engineering and Design, LEAP Program entails participating in activities hosted by �Ӱ�ԭ�� university, eating meals provided by your local establishments, attending and participating in related engineering activities as determined by the organizers.

To participate in the �Ӱ�ԭ�� Engineering and Design LEAP Program and related activities, the Participant (or Parent or Guardian) acknowledges and agrees as follows:

Medical Conditions
1. I agree to advise the organizers of any existing medical conditions or injury prior to the start of the program, so that this information can be shared with first responders in the case of a medical emergency.

Code of Conduct
1. I agree/agree that my child/dependent will not to consume any alcohol or illegal substances for the duration of this event.

2. I will behave/my child/dependent will behave responsibly and appropriately and adhere to any rules of conduct laid down by the organizers of this visit. Any incidentals acquired from the hotel will be covered by the child/dependent/parent/guardian and should be paid by credit card.

3. I have read/my child/dependent has read and understands the Charter of Responsibilities set out for the event.

If my child/dependent is late/misses the free transportation offered from Toronto, I am responsible for finding alternate transportation arrangements, and the cost associated with those arrangements.

Photo Consent
1. As an applicant or parent/guardian of an applicant of LEAP, I hereby give my permission to �Ӱ�ԭ�� University, to use my photograph or photographic image in official University business, including: University Web sites, University newsletters and newspapers, etc.

It is agreed that the use of my photograph or photographic image shall in no way be used in any other forum other than for Official University business. In consideration of my engagement as a model, and for other good and valuable consideration I herein acknowledged as received, I hereby grant to �Ӱ�ԭ�� University and assigns, those acting with its authority and permission, the irrevocable and unrestricted right and permission to copyright, in its own name or otherwise, and use, re-use, publish, and re-publish
photographic portraits or pictures of me or which I may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations, in conjunction with my own or a fictitious name, or reproductions thereof in colour or otherwise, made through any medium, and in any and all media now or hereafter known for illustration, promotion, art, editorial, advertising, trade, or any other purpose whatsoever. I also consent to the use of any printed matter in conjunction
therewith.

I hereby waive any right that I may have to inspect or approve the finished product and the advertising copy or other matter that may be used in connection therewith or the use to which it may be applied. I hereby release, discharge, and agree to save harmless �Ӱ�ԭ�� University and assigns, and all persons acting under its permission or authority or those for whom acting from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.

I do hereby release and forever discharge �Ӱ�ԭ�� University, its officers, Employees, representatives, and assigns from any and all demands, cause of action and/or judgements of whatsoever nature of character, past or future, known or unknown, whether in contract or in tort, whether for personal injuries, property damage, payments, fees, expenses, or damages of any kind or nature, and whether arising from common law or statute, arising out of, in any way, the use of my photograph or photographic image for official University business.

This release contains the entire agreement between the parties and shall be binding upon and inure to the benefit of the successors and assigns of the undersigned.

I further acknowledge and agree that the terms of this Release Agreement are fully understood by me; that I have had independent legal representation in connection with this Release Agreement or the opportunity to obtain same; and that I have voluntarily entered into this Release Agreement.
